
WHAT IS CFSS?
Community First Services and Supports (CFSS) is Minnesota’s new, modernized program that replaces the traditional PCA program. CFSS gives you much more flexibility and control over your care. You can choose your own caregivers (including, in many cases, a spouse or parent), decide when services are delivered, and even manage your own budget if you prefer.
ARE YOU ELIGIBLE FOR CFSS?
You may qualify for CFSS if you:
Are a Minnesota resident
Have a medical or physical condition that requires assistance with daily tasks
Are eligible for Medical Assistance (MA) or certain waiver programs
Can self-direct or have a responsible representative to help (for Budget Model)
If you currently receive PCA services, you will automatically transition to CFSS during your next annual reassessment. Metro Home Health Care will guide you through every step.

WHAT CFSS COVERS
CFSS covers all the same services as PCA plus additional supports:
Activities of daily living
Bathing, dressing, grooming, toileting, eating
Instrumental activities of daily living
Meal prep, laundry, light housekeeping, shopping
Health-related tasks
Medication setup, blood glucose checks with training
Observation and redirection for behavioral needs
Assistive technology and supplies

PCA CLIENTS
Your services will not be interrupted during the transition. Your case manager will contact you before your annual reassessment to explain your CFSS options, including the possibility of paying a family caregiver.
